Throttle flat spot

I've noticed lately my car has developed an occasional flat spot when coasting and reaplying the throttle, it makes the car jerk slightly around the 3% mark, it doesn't matter which regen or mode its in. Any help or ideas would be great taa
 
I haven't noticed this exact issue but found several times that after a short bought of hard acceleration, when I next apply throttle, nothing happens - almost like all drive has been cut. I then have to fully release the throttle pedal and reapply, then all seems back to normal. It's a bit disconcerting.
 
It feels like a miss fire even though it not as there is no engine, it's quite annoying as it used to be smooth.
How many km have you done? I thought I had this too recently, but I thought it might have been my 3rd party accelerator pedal cover slipping down and not allowing full acceleration because it contacted the floor. Slipping it back up with my foot fixed it, but I have since secured the cover properly. I'll keep an eye out for these flat spots.

Its done 8000 miles, I might disconnect the 12V battery for a bit and see if that helps.

What about in OPD where you can't really coast ? I know you said you've tried all modes but you rarely need to lift fully off in OPD.
